From Ciudad Real and Puertollano to Barajas airport: AVE with a change or a direct car
Ciudad Real has what almost no other Castilla-La Mancha capital has: an AVE that reaches Atocha in under an hour. The problem is the rest of the way: from Atocha to T4 you change to Cercanías C-1 or C-10 via Chamartín, or to the metro with another change, and the whole trip from your front door easily passes two and a half hours. By road it is 220 km along the A-41 and the A-4, about 160 min without traffic, round Madrid on the M-40 or M-50. For a flight boarding before nine in the morning, before the first AVE has arrived, the transfer with a driver from 272,91 € is the only door-to-door option; from Puertollano it is 258 km and from 314,31 €.

Why does the AVE only half help for Barajas?
Ciudad Real station is on the Madrid–Seville line, with long-distance trains and Avant shuttles reaching Puerta de Atocha in around fifty-five minutes. It is by far the best public transport the province has towards Madrid. But Atocha is south of the city and the airport north-east: no train links the two in one go. The least bad option is Cercanías: Atocha to Chamartín on the C-1, C-2, C-7, C-8 or C-10, and at Chamartín the C-1 or C-10 to the "Aeropuerto T4" station. That is two more trains, two waits and a long walk through T4.
If the flight leaves from T1, T2 or T3, add Aena's free shuttle between terminals, about fifteen minutes more. Counting the trip from home to Ciudad Real station, the AVE access check and the two changes, a realistic total is two and a half to three hours from your front door, and it only works at times when there is an AVE: the first morning train is no use for boarding before nine, and at night the last AVE to Ciudad Real leaves Atocha before the late flights land. Timetables change by season and are at renfe.com.
What is the road like and where is time lost?
From Ciudad Real you take the A-41 to Puerto Lápice and there join the A-4, which runs straight up through Madridejos, Ocaña and Aranjuez. The A-4 enters Madrid from the south, but for Barajas there is no need: you take the M-50 or M-40 eastbound and go round the city to the A-2 and the airport junctions (M-14 for T1, T2 and T3; M-12 for T4). From Puertollano it is the same route with the full A-41, from Puertollano to Puerto Lápice via Ciudad Real: 258 km and about 184 min.
The stretch that changes most with the time of day is not in La Mancha but at the end: the entry into Madrid on the A-4 between Pinto and the M-40, which clogs between seven and half past nine in the morning towards Madrid and on Friday afternoons the other way. Beyond Ocaña, the R-4 toll road runs parallel and is usually empty up to the M-50; it is the alternative when the A-4 is heavy and the boarding time allows no delay. On Sunday evenings at the end of a long weekend, between six and ten, the A-4 towards Madrid is the worst road in the region.
Is there a bus from Ciudad Real to the airport?
Not directly. Intercity lines from Ciudad Real reach the Estación Sur at Méndez Álvaro, in Madrid, and from there you continue to the airport by metro (line 6 to Nuevos Ministerios and line 8 to the terminals, with the airport supplement) or by Cercanías. The bus takes longer than the AVE and costs less; adding the urban leg, reaching T4 from Ciudad Real by bus is a trip of more than three and a half hours that only pays off for someone travelling light with no fixed time.
From Puertollano, Almagro, Daimiel or Miguelturra the bus first adds a connection to Ciudad Real or to Madrid, with timetables designed for the working day, not for flights. So in this province the real question is not "AVE or bus" but "AVE with three changes or a direct car", and the answer depends on the flight time, the luggage and how many are travelling.
When is the transfer with a driver worth it?
For flights boarding before nine in the morning, always: the first AVE does not arrive in time and the alternative is sleeping in Madrid. For flights landing after eleven at night too, because the last AVE has already gone. And at any hour, when three or four people travel with suitcases: the fixed transfer price is shared, and the AVE, with individual tickets plus the Cercanías, stops being cheap as soon as the group grows. The "from" fare from Ciudad Real to Barajas is 272,91 € per car, not per person.
For one person with a backpack and a mid-afternoon flight, the AVE still wins: under an hour to Atocha and the rest by Cercanías with time to spare. What does not pay is mixing: taking the AVE and then looking for a taxi at Atocha for T4 costs more than the direct transfer from home and adds the uncertainty of Madrid city traffic, which is exactly what you wanted to avoid.
What time should you leave Ciudad Real for a flight?
The count runs backwards from boarding time, not from the plane's departure time. For a European flight with checked luggage, being in the terminal two hours before departure is reasonable; for an intercontinental one, three. Add the engine's time, 160 min from Ciudad Real in normal conditions, and a margin for the entry into Madrid that depends on the hour: half an hour at morning rush hour, less in the small hours, when the A-4 and M-40 are empty. A seven o'clock flight means leaving Ciudad Real at half past two or three in the morning; it is uncomfortable, but it is the only way to catch it without sleeping away from home.
When booking, what helps is giving the flight number and terminal, not just the time: the driver works out the pickup with check-in time and the traffic on that stretch, and on the way back follows the flight on the arrivals board, so a delayed plane does not turn into a paid wait or a car that leaves. At holiday departures — Easter, July, Christmas — the first flights of the day are the first to run out of cars in the province, and it pays to book days ahead.
